PESA/TESE (Sperm collection directly from the testis)
Surgical sperm extraction is used in case of the male partner have problem with sperm secretion, such as sterility, having one testis, semen secretion without sperm found, cannot secrete semen due to obstruction or congenital sperm duct. The doctor will directly collect or surgery from the flesh of testicles in order to find the sperm from semen tubes, or testicles and then used in fertilization process (ICSI)
Type of surgical sperm extraction
There are many types of surgical sperm extraction, doctor will decide the type of surgery as appropriate.
PESA (PERCUTANEOUS EPIDIDYMAL SPERM ASPIRATION)
PESA is a general treatment if no sperm found in the semen, doctor will use small needles penetrate through the scrotum into the main tube that collects sperm. After that doctor will suck the liquid that will be founded for sperm and evaluated the quality before going to fertilization process.
PESA surgery takes time about 10-20 minutes and can be used for local anesthesia or medication that can make deep sleep before the procedure. After the process done, the patient is suggested to rest about 1 hour and go back home.
TESE (TESTICULAR SPERM EXTRACTION)
In the case that no sperm can be found in the main tube that collects the sperm, doctor will cut a small piece of the testicles which having a small sperm tube to detect sperm. This method requires general anesthesia or make the patient sleep. The patient may have some pain which usually recover within 3 days.
Normally in surgical sperm extraction rarely find complications. Most of complications arise from bleeding, infection but seldom occur, or said that only 1% to happen. The patient may have a slightly pain or complications may occur from anesthesia injection like general surgery.
